1. Application form is duly filled up

2. i) Self-certified copy of Certificate of Incorporation along with Memorandum of Association and Article of Association / Partnership deed submitted ii) In case of Partnership or Proprietary concern, the same is indicated in the Udyam Registration/ certificate issued by MSME

3. Self-certified copy of the Udyam Registration/ certificate issued by MSME/ Department For Promotion of Industry and Internal Trade (DPIIT) Ministry of Commerce and Industry - adequately reflecting the requisite product code/ category is enclosed

4. Addresses mentioned in the application form are matching with addresses mentioned in various documents/ certificates

5. Product code/ category for which WMI code is desired is clearly indicated

6. Details of the Coding system currently being used, if any, submitted

7. Application fees remitted 8. Proposed production initiation date/ month

9. Is the application completed in all aspects
